Care Continuum Group
Program Location: Delaware

Care management services are provided to people caring for someone who needs assistance with long term care planning or the coordination of in-home services. Services may include coordinating home health care, housecleaning, or transportation to and from medical appointments.

In-Home Counseling for Older Adults
Program Location: Delaware

Mental health counseling for persons over 60 years old and their caregivers is provided under a grant from the County's Office of Services for the Aging. Services are provided in the home or can be arranged at local senior centers. Mental health services are also offered to individuals enrolled in the Pennsylvania Department of Aging Waiver Program who are eligible for nursing home placement, but have chosen to remain in their home.

Senior Outreach Services
Program Locations: Delaware, Montgomery

A subsidiary of United Health and Human Services, this program offers mental health counseling and education to residents of personal care homes, assisted living, and retirement communities. Group, individual, and family therapy is provided, as well as mental health training for the facilities' personnel.
